ENROLLMENT - GRADES 1-12:
Attendance Eligibility
1. Download only ONE copy of each package below per family enrolling in grades 1-12:
2. Health Services - Please scroll through and read each section for important health requirements. Download ONE Copy of each health form below for EACH child enrolling.
- Physical Exam Form (Revised) (This includes immunizations.)
- Prescription Medication Form
- Dental Exam Form
- Health Screening Permission Form
- Health History Form – (Only applies to grades 1-5)
2010-2011 KINDERGARTEN REGISTRATION
1. Download ONE copy of the package below per family for KINDERGARTEN enrollment:

(This package includes all health forms required.)
****Complete all forms (for enrollment year) and bring to the Administration Office, which is located behind Pine-Richland High School at 702 Warrendale Road in Gibsonia, during the scheduled time for your child's school as listed below.
8 a.m. - 1:30 p.m. each day:
Hance Elementary at PR ADMIN BUILDING - Mon., Feb. 22, 2010
Richland Elementary at PR ADMIN BUILDING - Wed., Feb. 24, 2010
Wexford Elementary at PR ADMIN BUILDING - Fri., Feb. 26, 2010
AM/PM placement DOES NOT get assigned on a first-come first-serve basis. All preferences are considered and then assigned. Student must be 5 years old before September 1, 2010 to enroll.
Please have the kindergarten enrollment package completed before arriving at your registration session.
DOCUMENTS REQUIRED AT TIME OF ENROLLMENT
- 2 Forms of proof of residency
- Child’s birth certificate
- Child’s immunization records
- Specialized Education Plan, if applicable (IEP & ER, GIEP & GWR)
