    1. Written instructions signed by a physician and a parent consent form will be required.  Forms are available from the School Nurse or Principal.
    2. All medications must be sent in the prescription-labeled container which cannot be returned daily.  (If necessary, parents should request a duplicate container from the pharmacists.)
    3. Over-the-counter medications, such as aspirin, cough syrup, etc., will not be administered except as prescribed by a physician on the form provided by the Nurse or Principal.
    4. The School Nurse will:

1.     Be informed immediately and review all new medications requests.

2.     Inform appropriate school personnel of the medication.

    1. If the School Nurse is not on duty in a child’s building at the time a medication is received and / or required, the Building Principal or his /her designee will be the responsible individual to:

2.     Administer the medication.

3.     Keep a record of the administration of the medication.

4.     Keep medication in a locked cabinet and under refrigeration when required, in a secured area.

    1. Unused medication will be returned only to the parent upon request.  Otherwise, prescriptions will be discarded after one week following the termination date of the prescription or at the conclusion of the school year.
    2. Student will be directed to report at specific time for medication.
  1. INJECTABLE MEDICATIONS
    1. Injectable medications will be given in extreme emergencies only.
    2. Instructions regarding injectable medications must adhere to the previously outlined regulations for oral medications.
    3. If the student is unable to self-administer the injectable medication, it will be given by the School Nurse.  If the School Nurse is not available, a paramedic should be contacted.

NO MEDICATION WILL BE ADMINISTERED AT SCHOOL UNLESS ALL REQUIREMENTS OF THE MEDICATION POLICY ARE MET.
