End-of-Year Health Information

Medication Pick Up 

If your child has medication stored at school, please pick up your child's medication before the end of the school day on June 5. Unless your child has a doctor's order to carry his/her Epi-Pen, inhaler, or diabetic supplies, medications will not be sent home with students. Please contact the school health office if you need to make alternate pick-up arrangements.  

Please note, as per the Pine-Richland School District's medication policy, all unused medication left will be discarded on the morning of June 6. Medications are not able to be kept for next school year.  

Medication at School
2025-2026 School Year

Medication orders from your child’s doctor must be renewed every school year. If your child will need medication, whether prescription or over-the-counter, during the 2025-2026 school year, please download the Medication Permission Form. Have your child's doctor complete the form over the summer, and bring the signed form along with the prescribed medication on the first day of school. Additionally, please check the expiration dates on any inhalers, EpiPens and diabetic supplies to ensure they remain effective throughout the school year.

If your student will be carrying his/her EpiPen or inhaler, in addition to the school district's Medication Permission Form (see above), please complete the Permission to Carry Epi-Pens/Inhalers/Diabetic Supplies Form and return to your building's school nurse.​ 

Food Allergies

If your child has a life-threatening food allergy and requires an EpiPen at school, please have your doctor complete this Food Allergy Action Plan and bring this completed plan with the medication on the first day of school. For grades K-6, we are asking that parents complete an Allergy Self-Management Checklist to be submitted at the beginning of the next school year. 

Screening Letters

Screening letters were uploaded to the parent portal. If you did not receive one or have any questions, please contact your child’s school nurse.  

Preparing for 2025-2026 School Year

Immunizations

Please be sure your child's immunizations are in compliance with Pennsylvania Department of Health & Allegheny County guidelines before the beginning of the school year.

Rising 7th and 12th Graders

Please note the immunization requirements for students going into 7th and 12th grade. These are required before the start of school. Please provide an updated immunization record via email, fax or mail to your child’s school health office. These updates must be received prior to the start of the school year. Students who are not in compliance may be excluded from attending.  

Physical Exams

Physical examinations are required for all students entering grades 6 and 11. The exam must be dated after September 1, 2024. Completed forms are due by October 1, 2025. Please submit the updated record via email, fax or mail to the school health office. You may use the Pennsylvania Department of Health private physical examination form or a printed copy of the physical with the doctor’s signature. If your child has had a sports physical, please also send a copy to the nurse when uploading it to the Healthy Roster.

Students without documentation of a physical examination will be scheduled for an exam with the school physician during school hours. Parents are welcome to attend the examinations with their students as scheduled. These requirements allow for exemption due to religious beliefs. Please contact the school nurse if you are requesting an exemption. 

Dental Exams

Dental examinations are required for all students entering grades 3 and 7. The exam must be dated after September 1, 2024. Completed forms are due by October 1, 2025. Please submit the updated record via email, fax or mail to the school health office. You may use the Pennsylvania Department of Health private dental exam form or a signed printout from the dentist.

Students without documentation of a dental examination will be scheduled for an exam with the school dentist during school hours. Parents are welcome to attend the examinations with their students as scheduled. These requirements allow for exemption due to religious beliefs. Please contact the school nurse if you are requesting an exemption.
