Home Education Program Requirements

In order to be compliant, the enrollment documents listed below (with the noted exceptions) are required to be submitted to the District prior to the commencement of a home education program.     

All paperwork should be submitted in person, or mailed to:

Drop-off hours are Monday through Friday, 9:00 am - 3:00 pm

Initial Enrollment/Annual Start of School

  • Notarized affidavits or Unsworn Declarations and required attachments—due by August 1
  • Educational Objectives (attachment)
  • Special Education Program Approval, if applicable (attachment)
  • Proof of Immunizations* or Statement of Exemption (attachment)
  • Physical Examination* (may be submitted separately, but not later than October 1)**
  • Dental Examination* (may be submitted separately, but not later than October 1 )**

*Following initial submission, the following information must be submitted at the specified grade levels:

  • updated immunizations for Grs. K, 1, 7, and 12
  • updated physical examinations for Grs. 6 and 11
  • updated dental examinations for Grs. 3 and 7
  • End of Year
  • Initial Enrollment/Annual Start of School Year

**Parents/Guardians may also exempt their child(ren) from medical and dental examinations.  This exemption must be submitted in writing to the district. (28 Pa. Code Section 23.45 and 24 P.S. Section 14-1419)

In order to be compliant, the enrollment documents listed below (with the noted exceptions) are required to be submitted to the District from the parent and/or private tutor prior to the commencement of a private tutoring program:         

  • Proof of Immunizations* or Statement of Exemption - due by August 1 (provided by parent/guardian)  
  • Physical examinations (provided by parent/guardian) (may be submitted separately, but not later than October 1)*  
  • Dental examinations (provided by parent/guardian) (may be submitted separately, but not later than October 1)*  
  • List of student(s), age(s), grade(s), home address(es), parent/guardian name(s) and contact information - due by August 1 (provided by private tutor)
  • All Private Tutor clearances  (pg. 10-11) - due by August 1 (provided by private tutor), must be submitted every five (5) years
  • Pennsylvania Teaching Certification and PPID # (provided by private tutor)
